Case management, whether as part of an ACT team or as a separate service, can provide a number of important functions, ranging from linkage/connections to other providers to help with finance, housing, shopping, and medications. Case management services typically have a rehabilitative focus with written goals and objectives that the service, and its client, is actively working toward.
Case management services are available through various state and private nonprofit agencies, as well as through some in-home nursing services. Funding for case management varies and may be covered by Medicaid, some insurances, and through some state-funded assistance programs.
